On approval of the standard contract form for the provision of paid medical services

Guided by Article 84 of the Federal Law dated 21 November 2011 No. 323-FZ “On the Basics of Health Protection of Citizens in the Russian Federation” and Article 39.1 of the Law of the Russian Federation dated 07.02.1992 No. 2300-1 “On Consumer Rights Protection”, and by the Decree of the Government of the Russian Federation dated 4 October 2012 No. 1006 “On Approval of the Rules for Provision of Paid Medical Services by Medical Organizations”,

I HEREBY ORDER:

  1. To approve the standard form of the contract for the provision of paid medical services by the Moscow division of the institution “FEDERAL SCIENTIFIC AND CLINICAL CENTER of specialized types of medical care and medical technologies of FMBA of Russia” in accordance with Annex No. 1 to this order.
  2. To approve the standard form of the act for acceptance of rendered paid medical services by the Moscow division of the institution “FEDERAL SCIENTIFIC AND CLINICAL CENTER of specialized types of medical care and medical technologies of FMBA of Russia” in accordance with Annex No. 2 to this order.
  3. To implement the aforementioned forms specified in clauses 1 and 2 of this order into the activities of the Moscow division of the institution “FEDERAL SCIENTIFIC AND CLINICAL CENTER of specialized types of medical care and medical technologies of FMBA of Russia” starting from 2018.
